Care Management Support Coordinator IV Team Lead - Texas Remote

Description

  • Direct administrative care management functions, including member outreach, inbound calls, and service scheduling
  • Act as a primary contact for members, providers, and staff regarding care plan questions, while maintaining accurate records
  • Conduct phone outreach to explain care plan next steps, answer scheduling questions, share resources, and provide ongoing education
  • Help members access health plan and community-based resources
  • Use advanced knowledge of assigned health plan programs, activities, and available resources
  • Handle complex member and provider questions, requests, and concerns as a frontline support resource
  • Recommend improvements to care management processes
  • Apply advanced expertise in care management support, care plans, and community resources
  • Onboard team members and manage daily administrative materials, including welcome letters, correspondence, and educational content
  • Refer highly complex inquiries and requests to management
  • Maintain non-clinical member documentation in compliance with state and regulatory standards
  • Support education and training initiatives
  • Complete additional assigned responsibilities while following organizational policies and standards

Requirements

  • High school diploma or GED
  • At least four years of related professional experience
  • Successful completion of a Texas DFPS background check
  • At least two years of care coordination experience serving diverse populations, including children, adolescents, adults, and older adults
  • Experience with or working knowledge of PSON and MNSF workflow processes
  • At least two years of experience with STAR Health, STAR Kids, STAR+PLUS, Duals, Medicaid, and/or Medicare programs
  • Strong documentation abilities with demonstrated audit performance in TruCare Cloud
  • Professional, diplomatic, adaptable, and detail-oriented approach in a fast-paced setting
  • Proficiency in Microsoft Office, including Excel, OneNote, PowerPoint, and Outlook
  • Strong problem-solving, organizational, time-management, and communication abilities
  • Ability to adjust quickly to shifting business priorities while remaining flexible and professional
  • Ability to clearly present data, findings, and recommendations to cross-functional stakeholders
