Case Management Analyst, Region 5 — Louisiana Remote

Aprašymas

  • Assess member needs, eligibility, care barriers, and intervention opportunities comprehensively.
  • Coordinate routine care, support, and education to help members improve their health outcomes.
  • Apply motivational interviewing to strengthen member engagement and encourage healthier lifestyle changes.
  • Help members make informed healthcare decisions and participate actively in their care plans.
  • Work with case managers, supervisors, medical directors, providers, and other healthcare professionals to address member needs.
  • Present cases in interdisciplinary conferences to help achieve optimal member outcomes.
  • Recognize and escalate concerns about quality of care through established channels.
  • Help members obtain suitable services, benefits, and community resources.
  • Use effective negotiation to secure services and support required to meet member needs.
  • Document case management activities accurately, promptly, and in compliance with requirements.
  • Manage a varied caseload while meeting productivity, quality, and performance targets.
  • Assist with implementing case management programs, projects, policies, and processes.
  • Use technology and multiple systems effectively for member care and documentation.
  • Organize work independently while meeting priorities and deadlines.

Reikalavimai

  • At least two years of experience in case management, behavioral health, social services, healthcare, or a related field.
  • Experience working with adults and/or adolescents.
  • Strong customer service orientation and excellent problem-solving abilities.
  • Excellent verbal, written, and telephone communication skills.
  • Proven motivational interviewing ability, including establishing rapport and trust by telephone.
  • High attention to detail and exceptional documentation skills.
  • Commitment to compliance, regulatory requirements, and quality standards.
  • Highly organized and self-motivated, with the ability to work independently with minimal supervision.
  • Strong time-management and multitasking skills.
  • Proficiency with computers and technology platforms used for case management and documentation.
  • Ability to work effectively with members, providers, and internal stakeholders.
  • Ability to travel up to 50% of the time within 60 miles one way.
  • Must reside in Louisiana.
  • Ability to work a variable schedule, including two evenings each week until 9:00 PM local time.
  • Ability to meet established performance and productivity targets.
  • Bachelor’s degree required in healthcare, human services, social work, psychology, sociology, public health, or a related field; preferred qualifications include Medicaid, waiver program, managed care organization, certified case manager, master’s-level, crisis intervention, utilization review, case management and discharge planning, QuickBase or similar platform, Microsoft Teams, Outlook, Word, Excel, SharePoint, and complex-population care coordination experience.
