Ceresti Health
Ceresti Health
Ceresti Health is a technology-enabled care company working with health plans and accountable care organizations to support families caring for people with dementia. Its program combines claims-based risk assessment, caregiver education, a tablet platform and weekly guidance from personal care navigators. The team helps caregivers follow care plans, address social needs and identify changes in a person’s condition early. Ceresti reports independently validated reductions in avoidable hospitalizations and medical costs per patient, and offers payers outcomes-based contracts with no upfront risk.
