  • Coordinate administrative care-management work, including member outreach, inbound calls, and service scheduling
  • Act as a primary contact for members, providers, and internal staff by addressing and resolving issues
  • Maintain accurate member documentation in line with applicable state and regulatory requirements
  • Contact members by phone about care-plan actions, resources, appointments, questions, concerns, and continuing education
  • Link members to health-plan services and community-based resources
  • Use knowledge of assigned health-plan processes and available resources to support care delivery
  • Handle initial member and provider questions, requests, and concerns
  • Explain care-plan procedures, requirements, and protocols
  • Assist with member onboarding and related administration, including welcome letters, correspondence, and educational content
  • Maintain non-clinical member records and supply records to providers when requested
  • Refer members to services that address Social Determinants of Health needs
  • Complete additional assigned responsibilities while following organizational policies and standards

  • High school diploma or GED
  • One to two years of related professional experience
  • At least two years conducting outbound calls with members or providers in a remote call-center setting
  • Two years processing healthcare or insurance authorizations and coordinating services for members or providers
  • Preferred: two or more years supporting adults aged 21 and older who receive Long-Term Services and Supports (LTSS)
  • Preferred: two or more years conducting member or provider outreach in a fully outbound call environment
  • Preferred: two or more years processing healthcare or insurance authorizations and coordinating member or provider services
  • Detail-oriented, adaptable, and comfortable learning through hands-on experience
  • Proficiency with web-based platforms and CMS systems
  • Able to work Monday through Friday from 8:00 a.m. to 5:00 p.m. or 8:30 a.m. to 5:00 p.m. Eastern Time
