  • Triage referrals, assess patients at intake and coordinate admissions.
  • Coordinate and monitor patient care across virtual and home-based programs.
  • Guide patients, families and carers with clinical support and education.
  • Work with referrers, treating clinicians, external providers and multidisciplinary teams.
  • Monitor clinical risks and deterioration, and escalate urgent care needs.
  • Support discharge planning, care transitions and continuity of care.
  • Coordinate treatment pathways, equipment, consumables and service needs.
  • Use digital platforms to coordinate care, monitor patients, schedule services and manage escalations.
  • Facilitate virtual ward rounds, complex case reviews and multidisciplinary care discussions.
  • Manage a shared caseload and help balance workloads and patient flow.
  • Maintain accurate clinical records, documentation and activity reports.
  • Contribute to quality improvement, service development and innovation.
  • Help implement and grow technology-enabled care models.

Талаптар

  • Tertiary qualifications in Nursing.
  • Current unrestricted AHPRA registration as a Registered Nurse.
  • At least five years of clinical experience in acute, community, subacute, chronic disease management or care coordination settings.
  • Strong skills in comprehensive assessment, clinical management and care coordination.
  • Experience with discharge planning and collaboration with internal and external healthcare providers.
  • Strong communication and relationship management skills.
  • Ability to work effectively in multidisciplinary teams across different locations.
  • Excellent organisation, time management and prioritisation skills.
  • Confidence delivering care through digital and technology-enabled models, including telephone and video.
  • Strong digital literacy and experience with clinical information systems and electronic health records.
  • A flexible, positive and collaborative approach, with a growth mindset.
  • Commitment to continuous improvement, patient safety and evidence-based practice.
  • Satisfactory pre-employment checks are required, including AHPRA registration verification, immunisation requirements, reference checks, right-to-work verification and relevant criminal history screening.
  • Postgraduate qualifications in a relevant clinical specialty are highly regarded.
  • Experience in virtual care, telehealth, Hospital in the Home, community nursing or care coordination programs is highly regarded.
  • Knowledge of private health insurance and healthcare funding models is highly regarded.
  • Experience with digital care platforms or remote patient monitoring technologies is highly regarded.
