Field Case Management Coordinator
Coordinate care for Aetna members with complex health and social needs across Illinois. Support care plans, benefits access, referrals, and member engagement.
Apraksts
- Assess members’ needs and eligibility using care management tools and available data.
- Recommend case-resolution strategies and review benefit plans, programs, and services.
- Identify high-risk factors and service needs, referring members to clinical case management or crisis support when appropriate.
- Coordinate assigned care-plan activities and track progress.
- Work with case managers, supervisors, Medical Directors, and other health programs to address barriers.
- Present cases at multidisciplinary conferences to support effective outcomes.
- Identify and escalate concerns about quality of care.
- Arrange suitable options and services that address members’ benefit and healthcare needs.
- Use motivational interviewing to encourage member participation and support lifestyle and behavior changes.
- Coach members and provide information and support for independent medical and healthy lifestyle choices.
- Participate in team meetings and mentor or train peers.
- Help members make informed healthcare decisions with their providers.
- Follow case and quality management processes in line with regulatory, accreditation, and company guidelines.
Prasības
- At least two years of experience in behavioral health, social services, or a related field relevant to the program.
- Confidence working independently from home and collaborating with teams using virtual tools.
- Ability to travel within an assigned area for in-person case management as directed by leadership or business needs.
- Strong analytical and problem-solving skills.
- Effective communication, organization, and interpersonal skills.
- Ability to work independently.
- Proficiency with Microsoft Word, Excel, Outlook, PowerPoint, and proprietary applications.
- Efficient computer skills, including navigating multiple systems and keyboarding.
- Bachelor’s degree or a non-licensed master’s-level clinician credential.
- Degree in behavioral health or human services, such as psychology, social work, marriage and family therapy, or counseling.
