Case Management Analyst – Louisiana Remote

CVS Health is hiring a remote Case Management Analyst in Louisiana to assess member needs, coordinate care, and connect members with appropriate services. The role supports improved health outcomes through education, advocacy, motivational interviewing, and resource navigation.

Apraksts

Assess members comprehensively to identify needs, eligibility, care barriers, and intervention opportunities
Coordinate member care, support, and education to promote better health outcomes
Apply motivational interviewing to strengthen engagement and encourage healthier lifestyle changes
Help members make informed healthcare decisions and participate actively in their care plans
Work with case managers, supervisors, medical directors, providers, and other healthcare professionals
Present cases in interdisciplinary conferences to help achieve appropriate member outcomes
Recognize and escalate quality-of-care concerns through established processes
Help members obtain suitable services, benefits, and community resources
Negotiate access to services and support required to address member needs
Document case management activities accurately, promptly, and in compliance with requirements
Manage a varied caseload while meeting productivity, quality, and performance targets
Assist with case management programs, projects, policies, and process implementation
Use technology and multiple systems to support member care and maintain documentation
Work independently while balancing organizational priorities and deadlines

Prasības

At least two years of experience in case management, behavioral health, social services, healthcare, or a related field
Experience serving adult and/or adolescent populations
Strong customer service focus with effective problem-solving ability
Excellent verbal, written, and telephone communication skills
Proven motivational interviewing ability, including building rapport and trust by telephone
High attention to detail and strong documentation skills
Commitment to compliance, regulatory obligations, and quality standards
Well organized and self-directed, with the ability to work independently with minimal supervision
Strong time management and multitasking skills
Proficiency with computer systems and technology platforms used for case management and documentation
Ability to work effectively with members, providers, and internal stakeholders
Willingness to travel up to 50% of the time within 60 miles one way
Must live in Louisiana
Availability for a variable schedule, including two evenings per week until 9:00 PM local time
Ability to meet defined performance and productivity standards
Bachelor’s degree in healthcare, human services, social work, psychology, sociology, public health, or a related field
Preferred qualifications include Medicaid, waiver program, or Managed Care Organization experience; Certified Case Manager certification; a master’s degree in a healthcare or related field; crisis intervention, utilization review, case management, or discharge planning experience; experience with QuickBase or similar platforms; proficiency with Microsoft Teams, Outlook, Word, Excel, and SharePoint; and experience coordinating care for complex populations
