Care Management Support Coordinator II - Texas Remote at Centene

Support Centene members remotely in Texas through care-plan outreach, scheduling, education, and resource coordination. Handle member and provider inquiries while connecting people with health-plan and community services.

Description

  • Provide administrative support for care management operations
  • Contact members by phone about care-plan follow-up, resources, appointments, questions, concerns, and education
  • Handle inbound calls as an initial contact for member and provider questions
  • Arrange services and address issues involving members, providers, and internal staff
  • Refer members to health-plan and community resources, including support for Social Determinants of Health needs
  • Assist with member onboarding by distributing welcome letters, correspondence, and educational content
  • Record and maintain non-clinical member information and share records with providers when needed
  • Use working knowledge of assigned health-plan programs and available resources
  • Follow applicable policies and standards while completing other assigned responsibilities

Requirements

  • High school diploma or GED
  • One to two years of related work experience
  • Familiarity with available benefits and local community resources
  • Capability to conduct outreach and manage incoming calls
  • Ability to arrange services and resolve concerns involving members, providers, and staff
  • Ability to document and maintain non-clinical member records in line with state and regulatory requirements
  • For Florida-Sunshine Health Plan, conduct all member interactions by telephone
