Bilingual Nurse Case Manager – Canada Remote

Coordinate Canadian patient support programs for pharmaceutical therapies as a bilingual Nurse Case Manager. Manage enrollment, clinical follow-up, reimbursement, adherence support, and adverse-event reporting.

Description

  • Coordinate patient support services for Sentrex Health Solutions programs
  • Oversee program support and act as the primary contact for assigned health care providers
  • Help patients access support programs and their prescribed drug therapies
  • Work with patients, insurers, pharmacies, physicians, and other health care professionals to secure medication coverage
  • Complete health care provider and patient registration and enrollment
  • Enter and process service-delivery data accurately
  • Respond to telephone and web inquiries about program services, diseases, and products
  • Deliver medical support, disease and therapy education, and disease-management guidance
  • Complete specialized training for assigned therapies
  • Provide inbound and outbound telephone support, maintenance assistance, and adherence follow-up
  • Apply case-management protocols covering clinical monitoring, data collection, follow-up, compliance, therapy interruptions or discontinuation, and adverse-event reporting
  • Partner with reimbursement specialists and physicians to obtain therapy funding and investigate alternative funding sources
  • Attend patient meetings and conferences when required
  • Identify and report adverse events within specified timelines
  • Protect the confidentiality of patient, client, and corporate information
  • Coordinate with physician offices, pharmacies, infusion or injection providers, and other parties to deliver services and resolve issues
  • Take part in continuing education activities
  • Create training materials and programs for new patient support program team members
  • Advocate for patients and health care providers throughout enrollment, coverage renewals, and reimbursement support
  • Review, analyze, and verify patient information for accuracy and completeness
  • Schedule required next steps to maintain continuity of service
  • Follow up and advocate until coverage decisions are received
  • Document adverse events in accordance with pharmacovigilance requirements
  • Follow Sentrex policies and procedures
  • Complete other duties assigned by the manager

Requirements

  • Hold RN or RPN registration in good standing with the nursing college in the applicable province
  • Bring experience in Patient Support Programs
  • Reimbursement experience for securing patient coverage is an asset
  • Neurology experience is a strong asset
  • At least two years of clinical experience is preferred
  • Be available to work flexibly between 8:00 a.m. and 8:00 p.m. EST
  • Protect confidential patient, client, and corporate information
  • Demonstrate excellent organization and the ability to build effective working relationships
  • Adapt effectively to organizational growth and change
  • Build trust and influence stakeholders at multiple levels
  • Communicate with impact, candor, and clarity
