Sentrex Health Solutions is seeking a bilingual Senior Field Nurse Case Manager to coordinate patient support programs for pharmaceutical manufacturers across Quebec. The role combines clinical education, case management, enrollment, funding assistance, adherence support, and collaboration with clinics and healthcare providers.

- Coordinate services for Sentrex patient support programs
- Oversee program support and serve as the main contact for assigned healthcare providers
- Support physician-led patient enrollment through clinic visits and form completion
- Register healthcare providers and patients in program systems
- Process data collected during service delivery
- Respond to phone and web inquiries about programs, diseases, and products
- Deliver clinical support and education to patients and healthcare providers
- Attend specialized pharmaceutical training sessions
- Provide inbound and outbound telephone support, including adherence follow-up
- Manage case protocols covering clinical monitoring, data collection, follow-up, compliance, treatment interruptions, discontinuation, and adverse-event reporting
- Collaborate with care navigators and physicians to obtain therapy funding and explore alternatives
- Attend patient meetings and conferences when required
- Report adverse events to Health Canada and Medical Information teams, and record them in source and third-party systems
- Protect confidential client and corporate information
- Coordinate patient and physician services with medical offices, pharmacies, infusion providers, and other stakeholders
- Complete ongoing internal and external continuing education
- Advocate for patients and healthcare providers through enrollment, coverage renewals, patient-journey updates, and reimbursement support
- Apply program protocols to maintain uninterrupted service delivery
- Travel to physician offices as needed
- Identify barriers and improve patient-care processes
- Complete other responsibilities assigned by the manager
आवश्यकताएँ
- Hold active Registered Nurse licensure in good standing with the nursing college for the applicable province
- Bilingual English and French proficiency is required
- Have at least two years of clinical experience
- Provide proof of liability insurance and a current CPR certificate
- Bring experience in oncology, gastroenterology, or rheumatology
- Demonstrate collaborative, practical, and effective communication
- Patient Support Program experience is considered a strong asset
- Work effectively in a team setting
- Travel within the assigned geographic region up to 75% of the time to support clinics
- Be available Monday through Friday between 8:00 a.m. and 8:00 p.m. EST
