Health-Related Social Need Program Coordinator - Portland, OR (Hybrid)

Leads IRCO’s Portland Metro Health Related Social Need housing and outreach programs while supervising case managers and coordinating program operations. Provides trauma-informed, participant-centered support focused on housing stability for immigrant, refugee, and community participants.

Description

  • Oversee daily operations for IRCO’s Portland Metro HRSN Housing and Outreach and Engagement programs
  • Supervise, mentor, and support case managers
  • Manage program implementation, staff workplans, caseloads, and case openings and closures
  • Track expenses, eligibility, invoices, deliverables, and reports, and communicate with funders as needed
  • Lead trauma-informed, participant-centered services aligned with long-term housing goals
  • Maintain confidential participant records, service documentation, and compliant case notes
  • Approve rental and utility assistance requests and coordinate related financial matters with Finance
  • Onboard employees, conduct performance evaluations, and recommend hiring or disciplinary actions
  • Audit participant files, data entry, reports, timesheets, and administrative work for accuracy and timeliness
  • Revise program protocols and procedures
  • Identify and share staff training and professional development opportunities
  • Keep community resource and referral information current
  • Develop partnerships across IRCO programs and with external service providers
  • Attend internal and external meetings and advocate for participant needs
  • Coordinate and deliver ongoing HRSN staff training
  • Create and present culturally and linguistically specific presentations
  • Complete additional duties and lead assigned projects

Requirements

  • Fluency in English is required; proficiency in another language is preferred
  • Successfully pass an extensive background check
  • Bachelor’s degree preferred in public health, social work, counseling, human services, or a related field
  • At least two years of social services experience, preferably in housing programs
  • Experience with Oregon Medicaid programs or billing and the Health Related Social Need Waiver is preferred
  • Knowledge of housing programs, housing law, tenant rights, and housing systems is preferred
  • Understanding of social determinants of health and their intersections with violence, poverty, and homelessness
  • Knowledge of trauma-informed care and experience with reflective supervision
  • Understanding of immigrant and refugee populations, their challenges, and related cultural considerations
  • Ability to build effective relationships with stakeholders, colleagues, and community members
  • Compassionate and effective verbal and written communication with strong active listening skills
  • Highly organized and self-directed, with the ability to multitask, adapt, and meet competing deadlines
  • Strong analytical, problem-solving, and decision-making capabilities
  • Ability to navigate stressful situations while balancing individual circumstances, organizational policies, and program goals
  • Ability to produce accurate, clear, and concise program reports
  • Proficiency in tracking outcomes and outputs and preparing, reviewing, or analyzing reports
  • Experience tracking and analyzing program expenses, processing payments and expense requests, and managing complex budgets
  • Ability to pass an extensive background check
