Coordinate care transitions for individuals receiving services.
Engage with callers as a key point of contact.
Assess mental health, medical, and social service needs.
Complete needs assessments and other appropriate tools.
Refer callers to medical and behavioral health care, community programs, and service providers.
Track progress toward individualized goals.
Help people access services promptly, including residential admissions when appropriate.
Work with individuals, care teams, community stakeholders, and multidisciplinary teams.
Take part in supervision and team meetings.
Stay informed about care partners, service providers, and system stakeholders.
Support residential admissions using the bed registry and by coordinating with external stakeholders.
Follow program, department, and company policies, and carry out assigned duties.

Талаптар

High school diploma, GED, or equivalent required.
A college degree in human services or a related field is strongly preferred.
One year of work or other experience supporting people with complex and changing behavioral health needs is preferred.
One year of community health experience or lived experience is preferred.
Knowledge of person-centered, evidence-based practices.
Ability to assess mental health, medical, social, and other service needs.
Ability to make referrals to medical and behavioral health care, community programs, and service providers.
Ability to work with multidisciplinary care teams and community stakeholders.
Availability for either Tuesday–Saturday or Sunday–Thursday, 11:00 p.m.–7:00 a.m. shifts.
No travel required.
