- Help benefit recipients navigate the product platform
- Recommend services based on members’ needs and capabilities
- Assist members with placing orders
- Use assessment results, screening information, and knowledge of community resources to support members’ care goals
- Handle administrative work that helps members plan, organize, and use their benefits
- Answer incoming calls and help members complete assessments
- Recommend service plans based on members’ needs and capabilities
- Call members to follow up
- Help members and families access home care, meals, transportation, errands, home modifications, pest control, and social support
- Coordinate and schedule services
- Meet targets for calls, quality assurance, after-call work, member satisfaction, attendance adherence, and testimonial submissions
- Carry out other assigned or necessary duties
आवश्यकताएँ
- A strong interest in improving health outcomes through home and community-based services
- Computer proficiency and familiarity with clerical tasks
- Strong interpersonal and phone communication skills
- Excellent written and verbal communication
- Experience handling inbound calls
- Ability to work with multidisciplinary teams
- Strong time-management and organizational skills
- Ability to work effectively in a fast-paced startup environment
- At least 18 years of age
- High school diploma preferred
- Experience in public or community health, or a related field, preferred
- Experience in social services, care coordination, healthcare navigation, public health, community organizing, or volunteer work
- Ability to remain at the assigned workstation throughout the workday
- Ability to operate a computer, phone, and Voice over Internet Protocol (VoIP) equipment continuously
- Reliable internet and a designated workspace suitable for professional calls and sensitive information
- Candidates selected for the role may be asked to complete a background check and/or drug screen
- English-Spanish bilingual ability preferred, but not required
