Community Wellness Advocate — Boston, MA (On-Site)

Support Boston seniors and vulnerable patients through home visits, care coordination, health education, and access to healthcare, benefits, and community resources.

Description

  • Initiate face-to-face contact with eligible patients, explain the program and participation benefits, and begin screening
  • Schedule and complete hospital, clinic, and community visits, screenings, care plans, follow-ups, and phone calls
  • Deliver educational messages using culturally, linguistically, and educationally appropriate approaches
  • Document activities, assessments, patient updates, and data in patient records, care management systems, and Epic
  • Support community outreach, presentations, material development, and telephone engagement
  • Partner with patients and providers to establish care goals and offer guidance
  • Reinforce disease self-management education by connecting clients with community services and programs
  • Present patient cases during review meetings
  • Consult Patient Care Managers, clinical staff, behavioral health teams, and primary care providers about complex patient situations
  • Work within an interdisciplinary team to connect patients with appropriate resources
  • Track participant progress toward established goals
  • Prepare required reports and documentation
  • Help patients organize records, arrange and attend follow-up appointments, and fill prescriptions
  • Assist patients with applications for Medical Assistance, housing, and SNAP
  • Provide advocacy, patient education, and warm handoffs to community and hospital programs
  • Help patients address barriers related to healthcare, behavioral health, financial assistance, childcare, housing, utilities, food, transportation, and other resources
  • Coordinate with community-based long-term services and supports
  • Conduct intensive home and community outreach, motivational interviewing, goal setting, resource connection, and appointment accompaniment
  • Visit patients in hospital and emergency department settings to support care transitions
  • Build culturally appropriate, trusting relationships with patients and families
  • Attend assigned training, supervision, and program meetings
  • Develop and maintain relationships with community organizations and resources
  • Do not provide hands-on care or home health services

Requirements

  • High school diploma with community experience or a bachelor’s degree
  • Valid driver’s license
  • Community Health Worker (CCHW) certification
  • At least two years of healthcare, public health, or community-based experience
  • Basic knowledge of the healthcare system
  • Strong interpersonal skills for working with patients and families
  • Interest in community health and outreach
  • Excellent organizational skills, with the ability to multitask and work independently and collaboratively
  • Strong spoken and written English communication skills
  • Haitian Creole or Spanish fluency preferred
  • Understanding of how language, culture, and socioeconomic conditions affect health
  • Commitment to working with diverse, multicultural, and multilingual populations
  • Proficiency with Microsoft Office applications, including Word, Excel, Access, and Outlook, as well as web browsers
  • Proficiency in data entry and data tracking
