Community Health and Wellness Services Coordinator

Coordinate trauma-informed community health programs at IRCO in Portland, supervising community health workers, peer specialists, and birth doulas. Manage billing, reporting, referrals, staff development, and participant services for immigrant and refugee communities.

Description

  • Manage daily operations for Community Health and Wellness programs
  • Supervise Community Health Workers, Peer Support Specialists, and Birth Doulas
  • Create staff workplans and oversee caseloads, case activity, expenses, and eligibility
  • Submit invoices, monitor deliverables, prepare reports, and liaise with funders
  • Lead case conferences, mentor employees, provide coverage, and assist with complex client situations
  • Deliver a participant-centered service model grounded in trauma-informed care
  • Maintain confidential participant files, service records, and case notes in line with award and billing requirements
  • Authorize client assistance and award expenditures in partnership with Finance
  • Onboard employees, conduct performance reviews, interpret policies, and recommend personnel actions
  • Promote a supportive, positive, and accountable team culture
  • Audit participant files, service documentation, data entry, reports, timesheets, and administrative work
  • Revise program protocols and procedures as needed
  • Identify and circulate training and professional development opportunities
  • Help staff maintain Continuing Education Units records and active credentials
  • Develop referral partnerships with IRCO programs and external service providers
  • Attend internal and external meetings and advocate for participant needs
  • Coordinate and deliver continuing staff training
  • Develop and present culturally and linguistically responsive presentations
  • Plan, deliver, document, and evaluate assigned programs and projects

Requirements

  • Professional fluency in English is required; additional language skills are preferred
  • Current Community Health Worker certification and at least two years of experience are required
  • Bachelor’s degree preferred in public health, social work, counseling, human services, or a related field
  • At least two years of relevant experience as a certified Community Health Worker is required
  • Experience and active certification as a Peer Support Specialist, Certified Recovery Mentor, Birth Doula, or another Traditional Health Worker are preferred
  • Familiarity with Oregon Medicaid programs and billing practices is preferred
  • Knowledge of how social determinants affect health and intersect with violence, poverty, and homelessness
  • Knowledge of Oregon healthcare systems, providers, and the Community Health Worker model
  • Understanding of trauma-informed care and experience with reflective supervision
  • Awareness of immigrant and refugee communities, their challenges, and the importance of cultural sensitivity
  • Ability to build effective relationships with stakeholders, coworkers, and community members
  • Compassionate, effective written and verbal communication with strong active listening skills
  • Highly organized and self-directed, with the ability to multitask, adapt, and meet competing deadlines
  • Strong analytical, problem-solving, and judgment skills
  • Ability to manage stressful situations while balancing individual needs, organizational policies, and program objectives
  • Ability to produce accurate, clear, and concise program reports
  • Proficiency in tracking outcomes and outputs and preparing, reviewing, or analyzing reports
  • Experience monitoring program expenses, processing payments and expense requests, and managing complex budgets
  • Ability to complete an extensive background check
  • Willingness to participate fully in required professional development and training
