Complex Case Coordinator - Detroit, MI Hybrid

Coordinate integrated behavioral health services for Detroit Wayne members across assessments, care transitions, community referrals, and compliance requirements.

Description

  • Oversee care coordination initiatives
  • Advance integrated service delivery across DWIHN, its provider network, and the broader healthcare community
  • Apply NCQA standards for Complex Case Management
  • Manage an assigned caseload of DWIHN members
  • Complete required MHWIN documentation within established deadlines
  • Meet members in hospitals and community-based settings
  • Complete assessments and care plans within required deadlines
  • Identify eligible members using inpatient hospital, EMS, PHQ, hospital recidivism, and other referral data
  • Lead outreach, education, and marketing efforts for Wayne County community providers
  • Process complex case management referrals within designated time frames
  • Prepare weekly and monthly reports
  • Support care transitions for members leaving acute care settings
  • Maintain consolidated member lists and data for reporting purposes
  • Coordinate with community-based member care teams
  • Link members with community resources and healthcare providers
  • Help members apply for health benefits, SSI, Bridge Card, and related assistance
  • Participate in care review mock audits and record reviews
  • Coordinate care for members who do not qualify for Complex Case Management
  • Attend weekly team, all-staff, DWIHN, designated CRSP/COPE/Children/Hospital Liaison meetings, and required training
  • Complete other related duties as assigned

Requirements

  • Bachelor’s degree in social work, psychology, counseling, nursing, human services, social services, public health, public administration, healthcare administration, health management, or a related discipline
  • Four years of full-time paid professional experience in human services, social services, mental health, or behavioral health
  • Licensed Bachelor’s Social Worker (LBSW) credential or a higher-level license
  • Valid Michigan driver’s license and a safe, acceptable driving record
  • Familiarity with DWIHN policies, procedures, operations, provider network, and community resources
  • Understanding of the Michigan Mental Health Code, MDHHS policies, and applicable federal policies, regulations, rules, and procedures
  • Knowledge of MHWIN, the LOCUS model, clinical care processes, adult continuums of care, integrated care services, Medicaid, Medical Necessity Criteria, ASAM, DSM-V, NCQA standards, CMHSPs, and PIHPs
  • Strong assessment, evaluation, detail management, time management, report writing, critical thinking, interpersonal, communication, active listening, computer, and teamwork abilities
  • Proficiency in Microsoft Word, Excel, Access, PowerPoint, Outlook, and Teams
  • Ability to communicate effectively in writing and verbally while working with diverse populations
  • Sound judgment and reasoning skills
  • Proof of full COVID-19 vaccination required for employment, subject to legally required accommodations or exemptions
