Bilingual Housing Navigator (San Diego, Hybrid)

Support unhoused members in San Diego by helping them secure stable housing and connect with shelters, landlords, care plans, and community services. Titanium Healthcare seeks a bilingual professional to coordinate housing resources, applications, assessments, and tenancy support.

Description

  • Identify and maintain current housing opportunities throughout the region
  • Build working knowledge of and partnerships with transitional housing programs, shelters, permanent housing providers, and other local resources
  • Create individualized housing plans and explain available options to members
  • Coordinate housing services and placements with each member’s care plan
  • Guide members through housing searches, apartment and rental applications, and landlord negotiations
  • Educate members about tenant rights and responsibilities
  • Facilitate communication between members and landlords when disputes or concerns occur
  • Complete follow-up meetings with members after they are housed
  • Document service objectives, outcomes, and member information accurately in the appropriate systems
  • Contribute to collaborative team meetings and the development of effective practices
  • Apply departmental and company policies and procedures consistently
  • Observe all safety and injury-prevention policies and regulations
  • Enter members into the housing portal through the Homeless Management Information System (HMIS)
  • Help members complete Coordinated Entry System (CES) assessments
  • Provide in-person assistance with assessments and housing-tenancy needs
  • Handle additional responsibilities assigned under departmental policy

Requirements

  • A high school diploma or GED is required; an associate degree is preferred
  • At least two years of experience in healthcare, social services, public health, case management, or programs serving unhoused, unsheltered, family, or veteran populations
  • Previous experience using HMIS
  • Experience in a social services environment, including familiarity with case management systems and planning methods
  • Fluent written and spoken English
  • Spanish bilingual proficiency
  • Clear communication skills for in-person, telephone, and electronic interactions
  • A current, valid driver’s license and clean driving record
  • Reliable transportation and proof of auto insurance
  • Current American Heart Association BLS certification by the start date
  • A secure internet connection and distraction-free home workspace
  • Proficiency with computers, email, virtual meeting platforms, and Microsoft Office programs
  • Ability to lift, carry, push, or pull 20–25 pounds
  • Ability to climb stairs and travel across uneven terrain
  • Ability to bend, reach, and conduct visits in non-traditional settings
  • Ability to drive and travel throughout the community
  • Ability to work independently and complete assignments under established policies and procedures
  • Ability to protect patient confidentiality and follow ethical healthcare standards
