  • Serve as the main client contact within the ECM program
  • Coordinate services among medical, behavioral health, and social service providers
  • Assess clients’ strengths, needs, and goals through interviews, screenings, and evaluations
  • Create individualized case plans in partnership with clients
  • Deliver individual education, life-skills instruction, workshops, and group activities
  • Tailor training approaches and materials to varied learning styles, cultures, and developmental levels
  • Help clients establish attainable life-skills goals and track their progress
  • Support client participation in care decisions while explaining rights, resources, and self-advocacy
  • Link clients with providers, agencies, community resources, support networks, vocational programs, educational classes, and social services
  • Stay informed about available resources and work with community partners
  • Offer emotional support, active listening, validation, problem-solving help, and access to crisis resources
  • Keep client records, case plans, case notes, and related documentation accurate and current
  • Prepare and submit reports covering client progress, service use, and outcomes
  • Protect confidential information and follow applicable regulations and agency policies
  • Work with interdisciplinary teams and take part in case consultations and multidisciplinary meetings
  • Complete other assigned duties and projects

  • High school diploma or equivalent is required
  • An associate or bachelor’s degree in social work, psychology, or a related field is preferred
  • At least one year of direct experience in behavioral health or social services is preferred
  • Computer proficiency, including Microsoft Office Suite and database and reporting tools
  • Strong verbal, written, and interpersonal communication skills
  • Effective time management, organization, and multitasking abilities
  • Strong accuracy and attention to detail
  • Ability to use discretion and protect sensitive information
  • Frequent driving is an essential job function
  • Valid California driver’s license and an acceptable driving record
  • At least two years of licensed driving experience
  • Eligibility for coverage under the organization’s motor vehicle insurance policy
  • Flexibility to meet client needs and attend required meetings, supervision, and program responsibilities, including possible weekends, holidays, split shifts, and evenings
