Remote Contract RN Care Advocate in the United States

Provide remote nursing support to Carewell members through health assessments, care coordination, personalized care plans, and advocacy. Help members navigate services, resources, and transitions in care.

Description

  • Assess members by phone and video, covering health status, medications, recent healthcare use, and social determinants of health.
  • Create personalized care plans with defined goals and next steps, revising them as member needs evolve.
  • Review medications and identify discrepancies, adherence obstacles, or possible interactions for provider follow-up.
  • Coordinate with primary care clinicians, specialists, pharmacies, DME providers, and home health agencies to address gaps in care.
  • Guide members through transitions of care, including follow-up after hospital and emergency department visits.
  • Refer members to community support for food, transportation, housing, and in-home assistance.
  • Explain provider guidance and treatment plans in accessible language, using teach-back to verify understanding.
  • Prepare members for appointments, clarify benefits, and support them in advocating for their own needs.
  • Identify clinical warning signs and refer urgent concerns to the appropriate provider on the same day.
  • Record all member interactions accurately within the applicable service-agreement deadlines.
  • Meet consent, HIPAA privacy, and documentation standards during every member interaction.
  • Advance care-gap closure, resource access, member experience, and timely, accurate documentation results.

Requirements

  • Hold an active, unrestricted RN license in at least one U.S. state.
  • A Nurse Licensure Compact multistate license is strongly preferred.
  • Reside in the United States.
  • Bring at least three years of RN experience in care management, case management, transitions of care, home health, or primary care.
  • Demonstrate the ability to build trust with older adults, including people who are skeptical, confused, or difficult to reach.
  • Apply sound clinical judgment and recognize when escalation is necessary.
  • Use EHRs, care management systems, and phone or video communication tools proficiently.
  • Learn new systems quickly.
  • Manage a member caseload and schedule independently with minimal direction.
  • Provide a reliable computer, high-speed internet, and private workspace suitable for HIPAA-compliant work.
  • Case management certification, such as CCM, is preferred but not required.
  • Experience with Medicare, dual-eligible members, or older adult populations is preferred but not required.
  • Telehealth or telephonic nursing experience is preferred but not required.
  • Experience using social determinants of health screening tools or navigating community resources is preferred but not required.
  • Bilingual communication skills are preferred, particularly Spanish.
