Apraksts

  • Coordinate communication among interdisciplinary care teams, including complex care navigators, health plan care managers, primary care providers, and specialists.
  • Arrange schedules for Complex Care Navigators.
  • Call patients with reminders.
  • Coordinate with health plan care managers.
  • Document all patient interactions accurately.
  • Assist outreach and navigation teams with operational and non-clinical tasks.
  • Answer patient calls and route escalated concerns to the appropriate team.
  • Determine which patient records are needed and request them from patients and community providers.
  • Complete other assigned duties.

Prasības

  • Speak Spanish and English fluently.
  • Have a high school diploma or GED, or higher.
  • Have experience resolving escalated customer service issues.
  • Know Microsoft Office, including Word, Outlook, PowerPoint, and Excel.
  • Use Excel to create, edit, and sort spreadsheets and apply basic analytical formulas such as VLOOKUP and IF.
  • Have experience with phone and fax systems.
  • Have experience using an electronic medical record (EMR) system.
  • Be located in the Central Standard Time (CST) zone.
  • Preferred: Associate degree or higher.
  • Preferred: Typing speed of at least 40 words per minute.
  • Preferred: Experience as a medical assistant, certified nursing assistant, pharmacy technician, or in another clinical or administrative role in a healthcare setting.
