Care Coordinator – Illinois Medicaid (Remote)

Support Humana’s Illinois Medicaid members through comprehensive assessments, individualized care planning, and service coordination. This remote field role involves extensive travel across assigned counties.

Description

  • Support Humana’s Illinois HealthChoice Medicaid Managed Care program
  • Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care environments
  • Assess members’ individual care needs through telephone and face-to-face evaluations
  • Help ensure members receive services in the least restrictive setting
  • Identify and address obstacles to effective care
  • Create and carry out personalized care-plan interventions
  • Coordinate services while monitoring and evaluating case management plans against members’ goals
  • Track member care and progress toward targeted outcomes on an ongoing basis
  • Respond to immediate and acute gaps in care and access
  • Help enrollees submit grievances and appeals
  • Explain MCO requirements, available services, and benefits
  • Provide information about and referrals to community resources
  • Direct members and families to appropriate care resources
  • Coordinate communication among payers, providers, interdisciplinary teams, and other care participants
  • Provide MLTSS or LTSS support according to business needs
  • Travel locally in the field 75–90% of the time to visit members
  • Report to the Manager, Care Management

Requirements

  • Bachelor’s degree in social sciences, social work, human services, or a related field; alternatively, an unrestricted Illinois LPN license with one year of comprehensive assessment and formal elder-care service experience, an LPC, or an LSW
  • Must live in Illinois
  • At least one year of healthcare and/or case management experience
  • At least one year of experience with Medicare or Medicaid recipients, long-term care, Home and Community Based Services, and/or managed care organizations
  • Intermediate to advanced computer proficiency
  • Experience using Microsoft Word, Excel, and Outlook
  • Experience with electronic information systems and software, including electronic medical records
  • Valid state driver’s license
  • Personal vehicle liability insurance that meets required limits
  • Completion of TB screening if selected
  • Availability to work Monday through Friday, 8 a.m. to 5 p.m. CST, with flexibility for business needs
  • Dedicated, interruption-free workspace suitable for protecting member PHI and HIPAA information
  • Home internet service with minimum speeds of 25 Mbps download and 10 Mbps upload
