Area Clinical Operations Manager, Texas (Hybrid)

Lead field clinicians and patient access for Esperta Health’s evidence-based in-home wound care across Texas. Oversee care quality, staffing, scheduling, and continuity across assigned markets.

  • Direct clinical operations across an assigned field area.
  • Manage and develop field clinicians through individual coaching, team meetings, performance reviews, and corrective guidance.
  • Put Clinical Operations priorities, policies, and workflows into practice consistently across the area.
  • Promote reliable processes for scheduling, documentation, communication, supplies, mileage, safety, and escalation.
  • Help clinicians complete onboarding, field integration, competency requirements, and preparation for independent practice.
  • Track patient demand, staffing levels, clinician schedules, and geographic coverage; resolve operational barriers and raise recruiting or coverage needs.
  • Review area scorecards to spot trends, coach performance, carry out improvement plans, and report patient risks, staffing, results, and next steps.
  • Track active patients for scheduling gaps and required follow-up or dressing-change visits.
  • Coordinate rescheduling after cancellations, missed or incomplete visits, and hospitalizations; manage care holds and post-discharge follow-up.
  • Address risks of interrupted care, patient disengagement, preventable discharge, or missed ordered visit frequency.
  • Assign initial in-home visits, NP follow-ups, dressing changes, and other visits to the appropriate clinical roles.
  • Work with Intake, Patient Care Coordination, Member Engagement, Medical Leadership, Education, and other teams to improve handoffs and clear barriers.
  • Track complete, timely documentation, including visit-note completion, medical necessity, procedures, supplies, and patient communications.
  • Act as the main operational escalation contact for field clinicians.
  • Refer changes in patient condition, safety concerns, scope-of-practice questions, and medical decision-making issues to the appropriate Medical Director.
  • Assist with audits, corrective action plans, payor requirements, regulatory readiness, patient complaints, and field service issues.

Reikalavimai

  • Bachelor’s degree in nursing, healthcare administration, business administration, or a related discipline, or equivalent relevant experience.
  • Current, unrestricted Registered Nurse license and a bachelor’s degree in nursing.
  • Five or more years in healthcare or clinical operations, field leadership, home-based care, or a related setting.
  • At least two years managing people directly, preferably licensed clinicians.
  • Experience using operational and clinical quality measures to manage performance and coordinate staffing, schedules, geographic coverage, and patient service.
  • Working knowledge of HIPAA, healthcare records, patient safety, and escalation protocols.
  • Strong organizational and communication abilities, including managing competing priorities and addressing performance concerns constructively and directly.
  • Ability to distinguish operational matters from issues requiring Medical Leadership and to establish structure in a growing, multi-state organization.
  • Availability to travel within the assigned area and join field visits or ride-alongs as needed.
  • Preferred: Background in wound care, home health, home-based primary or specialty care, or another mobile clinical service.
  • Preferred: Experience managing Nurse Practitioners, Registered Nurses, and Licensed Practical Nurses.
  • Preferred: Familiarity with Medicare, Medicare Advantage, commercial payors, and value-based care.
  • Preferred: Experience with clinician onboarding, competency checks, audits, corrective actions, and regulatory readiness.
  • Preferred: Experience supporting multi-state operations and rapidly expanding markets.
  • Preferred: Familiarity with BOP or a similar electronic health record, care management, scheduling, or task management system.
  • Preferred: Wound care certification or substantial provider-level or nursing experience in wound care.
