Remote Care Coordinator in Florida

Support behavioral-health patient intake, appointment coordination, and EMR documentation for new evaluations. Partner with licensed clinical staff to collect patient information and facilitate the intake process.

Description

  • Lead structured intake appointments for new patients and collect information needed for bio-psycho-social evaluations
  • Collect and record demographic details, presenting concerns, psychosocial and family history, education and employment history, previous behavioral-health care, medications, and basic health information
  • Enter intake findings accurately into the electronic medical record (EMR) to support licensed providers’ clinical evaluations
  • Partner with licensed clinicians throughout the intake and evaluation workflow
  • Prepare relevant patient history and intake details for provider review
  • Relay important intake information to the appropriate clinical team members
  • Arrange initial evaluation appointments with suitable providers
  • Confirm that intake forms, questionnaires, and consent records are completed before evaluations
  • Validate patient demographic and insurance details
  • Upload, organize, and maintain patient records in the EMR
  • Complete intake documentation within required timelines
  • Help create a welcoming, supportive experience for patients beginning care
  • Respond to basic questions about services, appointments, and intake procedures
  • Refer urgent concerns and clinical issues to licensed providers or clinical leadership
  • Protect patient confidentiality under HIPAA and follow organizational policies

Requirements

  • Bachelor’s degree required, preferably in psychology, social work, human services, healthcare administration, public health, or a related discipline
  • Entry-level position; experience in healthcare, behavioral health, social services, or customer service is preferred
  • Background in patient intake, scheduling, or care coordination is advantageous
  • Clear, effective interpersonal and communication skills
  • Ability to establish rapport with patients from varied backgrounds
  • Strong active-listening and interviewing abilities
  • Ability to gather sensitive information with professionalism and empathy
  • High level of accuracy and attention to detail in documentation
  • Ability to perform effectively in a fast-paced clinical setting
  • Basic computer skills and willingness to learn electronic medical record systems
  • Ability to protect patient confidentiality and comply with HIPAA requirements
  • Ability to maintain professional boundaries and understand the role’s scope
  • Ability to sit or stand for extended periods and operate standard office equipment
