Care Coordinator, Houston (Hybrid)

Coordinate referrals, resources, screenings, and patient support for DaVita’s chronic kidney care program. This hybrid role serves Houston-area dialysis clinics and home-based patients.

Description

  • Connect patients with eligible resources and support programs as part of care coordination
  • Help arrange referrals and appointments with healthcare providers
  • Make outbound calls and handle inbound inquiries from patients, providers, and Village resources
  • Complete non-clinical screenings and care pathways under manager direction
  • Refer complex matters to the Integrated Care Nurse when appropriate
  • Support the clinical team in guiding patients through the care continuum
  • Assess resource eligibility and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to enroll members in programs
  • Provide patients with prompt and professional communication
  • Keep documentation accurate, complete, and confidential
  • Communicate clearly and effectively in both written and verbal interactions
  • Support DaVita’s mission and values while delivering high-quality patient care

Requirements

  • High school diploma or GED is required
  • At least two years of experience in a professional office setting
  • Use discretion and sound judgment when handling confidential information
  • Strong verbal communication ability
  • Working proficiency with Microsoft Office, Excel, and Outlook
  • Experience using electronic medical records is preferred
  • Demonstrated commitment to service quality and patient satisfaction
  • Able to work independently while contributing effectively within a team
  • A quiet, private home workspace with dependable high-speed internet
  • Reliable transportation is required
  • One to three years of clinical medical office experience is preferred
  • CNA or medical assistant experience is advantageous
  • Background in care coordination, referrals, or care management is preferred
  • Familiarity with insurance payers is preferred
  • Experience providing direct patient care or support is preferred
