Wellness Director, Assisted Living and Memory Care
Lead clinical care, resident evaluations, and dementia programming at StoryPoint Red Cedar. Oversee the wellness team, regulatory compliance, care plans, and resident outcomes.
Descriere
- Oversee and coordinate clinical care services for residents.
- Lead the wellness team and hold it accountable for performance.
- Align care, pricing, and staffing through revenue management, resident evaluations, care conferences, and service plans.
- Coordinate the Alzheimer’s and Dementia Program.
- Apply and lead 1440 care standards.
- Communicate organizational, community, and wellness goals to team members, staff, and residents.
- Build relationships with community leaders and external partners, including skilled care providers.
- Promote wellness throughout the community with the Executive Director and Sales Team.
- Conduct admissions and evaluations every six months, after returns from hospital or skilled care, and when residents’ conditions change.
- Set measurable goals and outcomes for residents’ physical, mental, and psychosocial needs.
- Manage state regulations and audits to maintain community compliance.
- Obtain written orders for care plan changes.
- Check MARS/TARS and coordinate with the pharmacy to ensure medications and treatments are administered accurately.
- Review incident reports and decide whether assessments or care conferences are needed.
- Address identified personnel concerns.
- Communicate emergency policies and updates to staff.
- Support ongoing organizational improvement.
- Perform other assigned duties.
Cerințe
- Prefer an LPN or RN license registered in Michigan.
- CDP certification is required before hire or within 90 days of starting.
- Assisted living or memory care experience preferred.
- Leadership experience preferred.
- Demonstrated ability to manage revenue and financials successfully.
- Track record as a high performer.
- Ability to resolve complex issues and solve problems effectively.
- Availability for a flexible schedule, including on-call work, weekends, and holidays when needed.
- Ability to stand for extended periods; sit, stand, and walk; handle equipment and objects; and review detailed documents and computer screens.
- Ability to occasionally lift or move up to 25 pounds.
- Occasional travel between locations may be required.
