Post Acute Analytics is a healthcare technology company that helps health systems manage patients across the transition from acute care to post-acute services. Its cloud-based Continuum of Care Suite™ combines automated data capture, connections with acute and post-acute electronic medical records, predictive analytics, patient tracking, off-track alerts, and care-pathway creation. The platform, powered by Anna™, is designed to support utilization and care management for value-based arrangements such as bundled payments, accountable care organizations, and Medicare Advantage. Post Acute Analytics also provides clinical consulting, with a focus on improving visibility across care journeys, reducing avoidable readmissions, controlling total cost of care, and strengthening in-network utilization.
