Care Coordinator – New Mexico (Remote)

Coordinate behavioral health assessments, care plans, and interdisciplinary services for Magellan Health members in New Mexico. Help improve care quality, outcomes, and responsible resource use.

Description

  • Coordinate care for individuals and designated populations through assessment, planning, implementation, monitoring, and evaluation
  • Deliver virtual or in-person care coordination in line with contractual requirements
  • Promote effective use of clinical and financial resources to enhance care quality and member satisfaction
  • Support members with behavioral health conditions that require intensive intervention and ongoing oversight
  • Complete comprehensive health risk and needs assessments across psychosocial, physical, medical, behavioral, environmental, and financial factors
  • Create, communicate, document, and implement care plans while serving as the primary service-delivery contact
  • Coordinate and track strategies that support better health and quality-of-life outcomes for members and families
  • Recognize care gaps, address them where possible, and advocate for members' needs
  • Review care plans and evaluate the impact of interventions
  • Gather clinical path variance information to identify opportunities for improvement
  • Partner with members and interdisciplinary care teams to revise care plans as needed
  • Educate providers, staff, members, and families about care coordination and health management strategies
  • Support interdisciplinary collaboration and cost-conscious delivery of high-quality services
  • Work with members, caregivers, legal representatives, physicians, care providers, and ancillary support resources
  • Respond to member questions and concerns about care, providers, and delivery systems
  • Build and maintain professional relationships with inpatient, outpatient, and community resources
  • Prepare reports aligned with care coordination objectives
  • Support onboarding and mentoring for new team members when appropriate

Requirements

  • Bring 3–5 years of experience in social work, nursing, a healthcare-related field, or equivalent relevant experience in place of a degree
  • Have experience in utilization management, quality assurance, home or facility care, community health, long-term care, or occupational health
  • Analyze trends using decision-support systems
  • Apply business management skills such as cost-benefit analysis, negotiation, and cost containment
  • Understand referrals to community and private or public resources
  • Demonstrate detailed knowledge of cost-effective care coordination and data interpretation
  • Make decisions that require substantial analysis and investigation
  • Determine appropriate actions in complex situations beyond existing policies or protocols
  • Maintain complete and accurate enrollee records
  • Communicate effectively in writing and verbally
  • Collaborate effectively with clinicians, hospital officials, and service-agency contacts
  • Hold a GED or high school education
  • Maintain a valid driver's license issued by the state
  • Follow applicable legal, regulatory, contractual, and internal policy requirements
  • Understand, follow, and attest to role-specific security responsibilities and controls
