Remote Health Coach (NBHWC Certified)
Provide virtual chronic care coaching and support to Cadence Health’s Medicare patients. The role includes care coordination, health assessments, and remote patient outreach.
Opis
- Reach patients by phone to provide one-on-one coaching for chronic conditions such as type 2 diabetes, hypertension, and cardiovascular disease.
- Support patients in following personalized care plans through education on behavior change, nutrition, physical activity, and self-management.
- Apply evidence-based methods, including motivational interviewing and SMART goals.
- Assess patients’ health, lifestyle behaviors, nutrition habits, and readiness to make changes.
- Teach patients about disease management, medication adherence, symptom recognition, and prevention.
- Deliver care tasks in line with Cadence clinical policies and protocols under appropriate clinical supervision.
- Track patient progress, care plan adherence, and health outcomes through regular check-ins and remote monitoring tools.
- Record patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Identify care barriers, including social needs, and connect patients with relevant resources or internal support.
- Coordinate in real time with clinicians and patient support teams using the Cadence Platform, Google Workspace, and Slack.
- Share feedback to help improve workflows in a fast-changing environment.
Zahteve
- An active NBHWC health coaching certification is required.
- At least one year of experience as a health coach is required.
- Experience educating and supporting patients managing chronic conditions is required.
- Must be equipped to work remotely, including access to stable, high-speed Wi-Fi.
- Spanish fluency is strongly preferred.
- Proficiency with AI-supported clinical tools such as Gemini, Open Evidence, or similar platforms, and the ability to explain to providers and patients where AI can support care and where human judgment is essential.
- Skills in patient education, coaching, and care coordination.
- Accurate clinical recordkeeping and reliable follow-through.
- Ability to work independently in a remote, fast-paced setting with evolving workflows.
- Ability to use and troubleshoot multiple systems at the same time.
- Strong interpersonal communication skills for building trust with patients over the phone.
