Field Case Management Coordinator, Lake County, IL
Coordinate care, assess needs, and support CVS Health members through in-person visits across Lake County, Illinois. The role combines care planning, member education, and collaboration with clinical teams.

- Support members in achieving appropriate health outcomes through coordinated care, education, and guidance.
- Assess referred members’ needs and eligibility using care management tools and available data.
- Review benefit plans and relevant internal and community programs and services.
- Spot high-risk factors and service needs, referring members to clinical case management or crisis support as appropriate.
- Carry out assigned care plan activities and track each member’s progress.
- Work with case managers, supervisors, Medical Directors, and other health programs to address barriers to care.
- Bring cases to multidisciplinary conferences for review.
- Flag and escalate quality-of-care concerns through established channels.
- Arrange suitable services and options that align with members’ benefits and healthcare needs.
- Use motivational interviewing to encourage engagement and support lifestyle or behavior changes.
- Coach members and provide information that helps them make independent healthcare and healthy-living choices.
- Help members take an informed role in healthcare decisions with their providers.
- Follow case management and quality processes in line with regulatory, accreditation, and company standards.
Prasības
- Must live in Lake County, Illinois.
- At least two years of experience in behavioral health, social services, or human services.
- At least two years of case management experience.
- Able and willing to use a personal vehicle for travel up to 50–75% of the time to meet members in person in Lake County, Illinois, and nearby areas.
- Reliable transportation is required.
- A bachelor’s degree or a master’s degree as a non-licensed clinician is required.
- The degree must be in behavioral health or human services, such as psychology, social work, marriage and family therapy, or counseling.
- Discharge planning experience is preferred.
- Managed care experience is preferred.
- Microsoft Office experience is preferred.
