Referral Coordinator – Vero Beach, Florida (Onsite)

Coordinate patient referrals, prior authorizations, appointments, and medical records for Whole Family Health Center in Vero Beach, Florida. Serve as a key liaison among patients, families, providers, insurers, and specialists.

Description

  • Manage office communications, supplies, equipment, and patient service
  • Collaborate with clinical staff and connect patients with providers
  • Coordinate care and services between WFHC and external providers
  • Handle clerical tasks such as phone support, copying, faxing, and customer service
  • Schedule diagnostic tests and specialty appointments
  • Record referrals and patient information in charts and the electronic health record
  • Monitor referral status and documentation to support team communication and patient safety
  • Register patients using demographic and insurance details
  • Compile clinical history and referral materials
  • Share clinical information with specialists in line with referral requirements
  • Obtain prior authorizations and communicate with insurance companies
  • Provide medical details, including history, diagnosis, and prognosis
  • Supply medical information to financial services to support reimbursement
  • Explain referral details and expectations to patients
  • Guide patients and families through the healthcare system and serve as their primary contact
  • Work with insurance carriers to secure approval for required supplies and services when necessary
  • Follow up on referrals and scheduled visits
  • Request and monitor medical records and reports
  • Update patient charts with specialist consultations, hospital stays, emergency visits, and community resources
  • Participate in relevant educational programs
  • Complete additional assigned responsibilities

Requirements

  • High school diploma or GED is required
  • A college degree is preferred
  • Cross-training in electronic medical records, scheduling software, and Microsoft Outlook is required
  • One to three years of progressive experience in a medical office setting
  • Level 2 background clearance is required
  • Required vaccinations and a PPD test must be completed upon hire
  • Strong verbal and written communication skills
  • Spanish, Creole, or both are advantageous
  • Ability to manage multiple tasks in a fast-paced setting
  • Detail-oriented, patient-focused, and able to work with minimal supervision
  • Effective patient assessment skills
  • Ability to prioritize work and manage space and time efficiently
  • Critical and logical thinking skills
  • Occasional travel between locations may be required for coverage
