Engagement Care Coordinator – OhioRISE, Londonderry Onsite

Coordinate care for Ohio youth hospitalized with behavioral health needs through OhioRISE enrollment, discharge planning, and community referrals. Help families access behavioral health resources and maintain ongoing support.

Description

  • Coordinate immediate care needs for youth hospitalized with behavioral health conditions and enrolled in OhioRISE
  • Help families navigate the hospital discharge process
  • Explain the OhioRISE program to families and community partners
  • Refer families to follow-up care and support services
  • Participate in in-person and virtual meetings to complete engagement milestones
  • Create crisis safety plans, assessments, and individualized care plans
  • Arrange and lead Child and Family Team meetings
  • Coordinate family-centered services across homes, schools, and community settings
  • Assess family strengths, cultural considerations, care coordination needs, and suitable services
  • Establish supportive relationships with children, youth, families, and Child and Family Teams
  • Build collaborative relationships with community resource providers
  • Complete documentation and contribute to program evaluation, team and program planning, and cross-system training
  • Build knowledge of community resources and complete required training in High Fidelity Wraparound, Motivational Interviewing, and Cultural Humility
  • Carry out additional duties as assigned

Requirements

  • Provide services or support to children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services: three years with a high school diploma or equivalent, two years with an associate or bachelor's degree, or one year with a master's degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Training and experience demonstrating cultural humility or responsiveness in complex cases
  • Qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and child-serving systems
  • Strong organizational skills with the ability to manage competing priorities
  • Demonstrated strong cultural awareness
  • Experience supporting individuals and families with multiple needs
  • Broad understanding of community service systems
  • Willingness to participate in and lead cross-system care coordination
  • Strong verbal and written communication skills
  • Ability to work in an internet-based, automated office environment
  • Valid driver's license required
  • Ability to participate in video calls and meetings in an automated office environment
  • Minimum 100 Mbps high-speed internet connection required
