Case Management Coordinator – Field, Illinois

Support CVS Health members through telework and field-based care coordination in Cook County, Illinois. Assess needs, coordinate care plans, and help improve healthcare outcomes.

Description

  • Assess members’ needs and eligibility through care management tools and information review
  • Evaluate benefit plans and available programs to recommend solutions for member needs
  • Recognize high-risk factors and service needs, referring members to clinical case management or crisis intervention when appropriate
  • Coordinate assigned care plan activities and track member progress
  • Work with case managers, supervisors, Medical Directors, and other health programs to address barriers to care goals
  • Present cases for multidisciplinary review during case conferences
  • Escalate quality-of-care concerns through established channels
  • Arrange suitable options and services that address members’ benefits and healthcare needs
  • Engage members with influencing and motivational interviewing techniques to encourage lifestyle or behavior changes
  • Offer coaching, information, and support for independent medical and healthy lifestyle decisions
  • Attend team meetings and provide peer mentoring or training
  • Help members make informed healthcare decisions with their providers
  • Follow case management and quality management processes in line with regulatory, accreditation, and company guidelines

Requirements

  • At least two years of experience in behavioral health, social services, or a related field aligned with the program focus
  • Ability and willingness to travel 50–75% of the time throughout Cook County, Illinois, primarily Chicago’s West Side
  • Reliable transportation
  • At least two years of experience with electronic documentation
  • Experience using Microsoft Office applications
  • Bachelor’s degree or non-licensed master’s-level clinician qualification in behavioral health or human services, such as psychology, social work, marriage and family therapy, or counseling
  • Case management and discharge planning experience preferred
  • Managed care experience preferred
