Assess members’ needs and eligibility through care management tools and information review
Evaluate benefit plans and available programs to recommend solutions for member needs
Recognize high-risk factors and service needs, referring members to clinical case management or crisis intervention when appropriate
Coordinate assigned care plan activities and track member progress
Work with case managers, supervisors, Medical Directors, and other health programs to address barriers to care goals
Present cases for multidisciplinary review during case conferences
Escalate quality-of-care concerns through established channels
Arrange suitable options and services that address members’ benefits and healthcare needs
Engage members with influencing and motivational interviewing techniques to encourage lifestyle or behavior changes
Offer coaching, information, and support for independent medical and healthy lifestyle decisions
Attend team meetings and provide peer mentoring or training
Help members make informed healthcare decisions with their providers
Follow case management and quality management processes in line with regulatory, accreditation, and company guidelines

Талаптар

At least two years of experience in behavioral health, social services, or a related field aligned with the program focus
Ability and willingness to travel 50–75% of the time throughout Cook County, Illinois, primarily Chicago’s West Side
Reliable transportation
At least two years of experience with electronic documentation
Experience using Microsoft Office applications
Bachelor’s degree or non-licensed master’s-level clinician qualification in behavioral health or human services, such as psychology, social work, marriage and family therapy, or counseling
Case management and discharge planning experience preferred
Managed care experience preferred
