Tavsif

  • Assess members’ needs and care requirements by phone.
  • Connect members and families with appropriate care and wellbeing resources.
  • Address members’ physical, environmental, and psychosocial health concerns.
  • Identify and resolve obstacles to effective care.
  • Track patient progress through assessments and evaluations against desired outcomes.
  • Develop care plans for members when needed.
  • Conduct health assessments by phone.
  • Provide care planning and education about chronic conditions and gaps in healthcare plans.
  • Choose appropriate work methods independently within established guidelines and procedures.
  • Serve an assigned member population.

Talablar

  • Bilingual in English and Spanish
  • Bachelor’s degree
  • 3–5 years of clinical acute care experience
  • Active Registered Nurse (RN) license in the appropriate state, with no disciplinary action
  • Broad knowledge of Microsoft Word, Excel, and Outlook
  • Commitment to helping an organization continually improve consumer experiences
  • A dedicated work area with minimal interruptions to protect member protected health information (PHI) and comply with HIPAA
  • Internet service provided by the employee, with at least 25 Mbps download and 10 Mbps upload speeds
  • Availability to work between 8:00 a.m. and 7:00 p.m. Central Time, with core hours mainly from 8:00 a.m. to 5:00 p.m.
  • Occasional travel to Humana offices for training or meetings may be required
