Opis

  • Build partnerships with community organizations, schools, faith groups, coalitions, and local stakeholders.
  • Represent Humana at community events, health fairs, meetings, conferences, and stakeholder forums.
  • Identify collaboration opportunities that support organizational priorities, health outcomes, and population health strategies.
  • Connect internal teams with external partners to address community needs and improve access to services.
  • Support outreach that strengthens brand awareness and member retention.
  • Use public health principles and social drivers of health to identify barriers affecting members and communities.
  • Track community trends, disparities, and emerging needs to guide outreach priorities and organizational strategy.
  • Develop and manage a partner portfolio that supports member referral pathways.
  • Manage allocated budgets and resources responsibly.
  • Record outreach, events, referrals, and partner engagement in QuickBase or other tracking systems.
  • Maintain accurate data to support reliable records and reporting.
  • Travel across the assigned region for events, presentations, coalition meetings, and outreach.
  • Keep current on plan benefits and Florida’s Medicaid landscape.
  • Present to community groups in line with approved communications and marketing plans and state regulations.

Zahteve

  • Bachelor’s degree.
  • At least one year of experience in corporate social responsibility, community outreach, education, or nonprofit work.
  • At least one year of experience supporting Medicaid health plans or underserved populations.
  • Familiarity with Florida’s geography and culture, especially Northeast Florida.
  • Residence in Duval, Clay, Nassau, Baker, or St. Johns County.
  • Proficiency with administrative and data management systems.
  • Knowledge of community resources and how to navigate them.
  • Strong knowledge of community and faith-based organizations, social determinants of health, and health equity.
  • Strong presentation skills.
  • Understanding of marketing, social media, and public relations as they relate to field-based work.
  • Experience in event, project, or program management.
  • Home internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
  • A dedicated home workspace without ongoing interruptions to protect member PHI and HIPAA information.
  • Willingness to commute across surrounding counties.
  • Availability to work evenings and weekends as needed.
  • Ability to accommodate occasional overnight stays.
  • Ability to independently set up, assemble, transport, operate, and dismantle event materials and equipment weighing up to 20 pounds.
