Humana is hiring a Field Service Coordinator to assess Indiana members’ needs and coordinate healthcare, LTSS, and community resources. The role supports Medicaid eligibility, service planning, and positive health outcomes.

- Assess member needs and requirements to promote optimal wellness
- Visit members in their communities to evaluate needs, barriers, goals, and priorities
- Perform initial and ongoing long-term services and support (LTSS) assessments using person-centered approaches
- Engage with members by telephone and in person
- Create and regularly update service plans
- Include care-team members in planning activities
- Identify LTSS providers and build community partnerships
- Connect members with LTSS, environmental, social, housing, educational, and other services
- Help members maintain Medicaid eligibility
- Use available member information to avoid duplicate assessments
- Coordinate with Medical Directors, geriatricians, and care coordinators to deliver cohesive services and achieve positive outcomes

- Bachelor’s degree in a health and human services discipline
- At least two years of related experience
- Working knowledge of Microsoft Word and Excel
- Excellent verbal, written, and interpersonal communication skills
- Residence in Cass, Fulton, Howard, Miami, Tipton, or Wabash County, Indiana
- Preferred experience documenting electronic case notes and working across multiple computer applications or systems
- Preferred background in health promotion, coaching, or wellness
- Preferred knowledge of community health and social service agencies and local resources
- Home internet service with minimum speeds of 25 Mbps download and 10 Mbps upload
- Dedicated, interruption-free workspace that safeguards member PHI and HIPAA information
