Clinical Coordinator RN – Physician Practice (Onsite)

Clinical Coordinator RN responsible for clinical operations, care coordination, staff competency, and patient-focused initiatives in physician practices. This onsite position also supports office administration and care transitions.

Description

  • Direct clinical operations and patient engagement through visit flow, triage, EMR tools, pre-visit planning, transitions-of-care follow-up, and related initiatives
  • Take on clinical and administrative duties when the Director or Manager is unavailable
  • Complete daily closeout work, including reports, no-show follow-up, staff communication, task batching, and office closing procedures
  • Prepare daily patient charts and maintain paper and electronic medical records
  • Seek policy guidance as needed and coordinate with other departments for additional resources
  • Evaluate clinical staff competencies and arrange remediation with Clinical Services
  • Coordinate care across settings, track labs, studies, and referrals, and ensure results are reviewed with patients
  • Give ongoing feedback and involve team members in developing quality-improvement workflows
  • Promote staff accountability for organizational policies, procedures, and professional standards
  • Apply research findings in practice while supporting practice and hospital goals
  • Perform scheduling, reception, front-end revenue-cycle, medical assistant, and other physician-office duties
  • Act as a clinical resource while supporting staff orientation, education, competency development, operations, and remediation

Requirements

  • Associate degree in nursing from an accredited nursing school
  • Three years of recent clinical-setting experience
  • Ability to initiate, accept, and adapt to change in a healthcare environment
  • Ability to work independently, take initiative, and collaborate effectively
  • Strong computer skills, including EMR systems, Microsoft Word, PowerPoint, and Excel
  • Effective time-management skills
  • Mature emotional intelligence
  • Project-management skills
  • Active Pennsylvania Registered Nurse license
  • Ability to lift and carry up to 25 pounds
  • Frequent sitting, standing, and keyboard use
  • Patient-care providers may need to kneel, bend, squat, and perform CPR
