Care Coordinator – OhioRISE (Onsite, Ohio)

Coordinate family-led care teams and individualized plans for Ohio youth with complex behavioral health needs. Connect families with community resources while supporting stability at home, school, and in the community.

Description

  • Build and organize care teams according to family preferences
  • Plan, lead, and facilitate meetings for youth and their designated support teams
  • Deliver care coordination in homes, communities, or other locations chosen by families
  • Work with youth, families, natural supports, and service providers to identify care needs and appropriate services
  • Promote youth and family participation, voice, and choice in coordinated services
  • Maintain documentation required to obtain and retain OhioRISE benefits
  • Comply with ISBH OhioRISE quality and fidelity standards
  • Keep current knowledge of available community resources
  • Attend required in-person and virtual training and supervision, including travel as needed
  • Complete assigned training and maintain required certifications
  • Follow confidentiality, professional conduct, incident reporting, HIPAA, mandated reporting, and ethical requirements
  • Use databases and electronic health record systems to document client information accurately
  • Travel throughout the assigned service area
  • Complete other duties as assigned

Requirements

  • Required experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services: three years with a high school diploma or equivalent, two years with an associate or bachelor's degree, or one year with a master's degree or higher
  • Knowledge of and experience with High-Fidelity Wraparound preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination requiring strong efficiency, organization, and attention to detail preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family and community systems, community resources, case management, child and family counseling or therapy, child protection, or child development required
  • Required qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong verbal and written communication skills required
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings required
  • Ability to protect confidential information required
  • Ability to follow all safety rules, regulations, and applicable requirements required
  • Ability to bill Medicaid without restrictions required
  • Valid driver's license and ability to operate a motor vehicle required
  • Required auto insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume environment required
  • Ability to work in an internet-based, automated office environment required
  • Ability to participate in, receive, and conduct on-camera video calls and meetings required
  • Minimum 100 Mbps high-speed internet connection required
  • Ability to sit for extended periods while working at a computer required
  • Ability to lift up to 15 pounds occasionally
