- Assess members’ needs and support their wellness.
- Meet members where they live to identify needs and barriers.
- Connect members with services and resources that support their health and well-being.
- Conduct initial and ongoing LTSS assessments using person-centered approaches.
- Contact members by phone or in person to set goals and priorities.
- Review available resources, create care plans, and identify LTSS providers and community partners.
- Update service plans as needs change and involve relevant care team members in planning.
- Help members address LTSS, environmental, social, housing, educational, and other service needs.
- Support members in maintaining Medicaid eligibility.
- Work with the Medical Director, Geriatrician, and Care Coordinator to coordinate services and support positive outcomes.
- Use available member information to avoid repeating assessments.
Zahteve
- Bachelor’s degree in a health and human services field.
- At least two years of related experience.
- Knowledge of Microsoft Word and Excel.
- Strong verbal and written communication and interpersonal skills.
- Ability to use a range of electronic information applications and software.
- Complete required TB screening for this patient-facing role.
- Valid state driver’s license.
- Personal vehicle liability insurance that meets state minimums or Humana’s specified limits, whichever is higher.
- A separate room with a locking door that can be used as a home office.
- Ability to commute daily to a covered Indiana county: Benton, White, Fountain, Clinton, Tippecanoe, Warren, or Carroll.
- Medicare and Medicaid recipient experience preferred.
- Experience documenting electronic case notes and using multiple computer applications or systems preferred.
- Experience in health promotion, coaching, and wellness preferred.
- Knowledge of community health and social service agencies and other local resources preferred.
- Home internet with at least 25 Mbps download and 10 Mbps upload speeds.
