Care Partner – Community Aged Care (Onsite)

Provide telephone-based care management for Wesley Mission Queensland’s Community Aged Care customers. Support personalised care plans, budgets, safety, independence, and wellbeing.

Description

  • Provide care management for Support at Home customers
  • Create and maintain continuity of care through care plans, service coordination, and outcome tracking
  • Communicate mainly by telephone with customers and Community Aged Care stakeholders
  • Arrange personalised services based on customer goals, preferences, and approved care plans
  • Administer customer budgets within applicable funding limits
  • Identify changing needs and emerging risks, then take timely action
  • Develop effective relationships with customers, carers, supporters, and health professionals
  • Use wellness and reablement practices to promote customer independence
  • Reflect cultural, spiritual, and diverse needs in care planning
  • Help customers make informed decisions and manage positive risks
  • Keep documentation accurate, complete, and compliant
  • Follow the Aged Care Act 2024 and Strengthened Aged Care Quality Standards

Requirements

  • Certificate III in Individual Support (Ageing), Certificate IV in Aged Care, or an equivalent qualification
  • At least 12 months’ experience in aged care, disability services, or community services
  • Confidence using digital tools and client management systems
  • Strong communication and relationship-building abilities
  • Capacity to work independently while contributing to a supportive team
  • Effective time management and organisation to meet deadlines
  • A commitment to person-centred care
  • Proof of COVID-19 vaccination status, where required by legislation or WMQ policy
  • Completion of probity checks required by legislation or WMQ Limited policy
  • Current driver’s licence and willingness to travel for work
