Senior Contract Care Coordination Team Lead — Tennessee Remote

Lead clinical care coordination for Centerstone’s military behavioral health services in a remote Tennessee role. Supervise staff, manage referrals and compliance, oversee audits, and coordinate with providers.

Description

  • Lead the daily operations of the Clinical Care Coordination team
  • Conduct regular supervision sessions and facilitate team meetings
  • Oversee referral triage, case conferences, documentation reviews, training compliance, and productivity tracking
  • Coach, mentor, and support the professional development of team members
  • Track and evaluate care coordination performance metrics
  • Complete monthly audits and deliver timely internal reports to the Program Manager
  • Keep case status records for assigned referrals accurate and current
  • Support hiring, onboarding, and training for clinical care coordinators and operational support staff
  • Provide clinical care coordination and caseload coverage when needed
  • Handle manager-assigned requests and departmental operations, including phone lines and routine daily tasks
  • Support external provider consultation and coordination, including service continuation reviews and processing
  • Respond to routine outreach inquiries about services and referrals

Requirements

  • Master’s degree in social work, psychology, or a related field is required
  • A bachelor’s degree plus at least three years of leadership experience may be considered
  • Prior supervisory experience is required
  • Military affiliation is required
  • Applicants must be a current or former service member, the spouse or family member of an active-duty service member or veteran, or a Gold Star spouse or family member
  • At least two years of behavioral health or social services experience within the military community is required
  • Proficiency with Microsoft Office 365, CRM platforms, Zoom, basic exploratory data analysis and descriptive statistics, and Adobe
  • Strong written, verbal, in-person, and virtual communication skills
  • Ability to establish and achieve multiple short- and long-term goals simultaneously
  • Ability to drive effective change within a team environment
