Community Liaison and Resource Navigator

Connect Illinois members with housing and social services through Telligen’s tailored care management program. Support care managers and coordinate transitions for people with complex needs.

Description

  • Connect members with community programs and provide nonclinical support to care managers.
  • Build and maintain partnerships with community organizations, facilities, state agencies, and other local entities.
  • Refer members to transition services in line with their person-centered service plans.
  • Track referrals, follow up on outcomes, and coordinate activities within established timelines.
  • Direct members to relevant information about their benefits.
  • Keep an accurate, comprehensive resource library for members in Cook County and elsewhere in Illinois as needed.
  • Coordinate healthcare and transition services with care managers, healthcare professionals, and social service agencies.
  • Escalate issues through the appropriate channels and reporting structure.
  • Work across disciplines to support enrollee compliance and appropriate use of community resources.
  • Help members, families, and community supports access personal and environmental resources.
  • Carry out other duties as assigned.

Requirements

  • High school diploma or equivalent plus three years of community-based services experience, or a bachelor’s degree in social work or a related field.
  • Certification as a care or case manager, licensure as a practical nurse (LPN), health coach status with at least one year of experience, or Community Health Worker certification through a college-level program or ability to obtain it.
  • Strong understanding of social determinants of health and their effects on healthcare.
  • Valid Illinois driver’s license.
  • Reliable transportation.
  • English and Spanish bilingual skills preferred.
  • Behavioral health knowledge preferred.
  • Knowledge of confidentiality practices, HIPAA, and protected health information (PHI) requirements.
  • Ability to learn software tools, including database management and report development.
