Care Coordinator, OhioRISE (Onsite)

Coordinate OhioRISE teams and care plans for youth with complex behavioral health needs. Connect families with community resources that support stability and help prevent hospitalization.

Tavsif

  • Organize and facilitate child and family team meetings.
  • Deliver care coordination in families’ homes, communities, or other preferred settings.
  • Collaborate with youth, families, natural supports, and service providers to identify care coordination needs and services.
  • Support youth in their homes, schools, and communities.
  • Help preserve family unity and reduce hospitalizations.
  • Ensure youth and families have meaningful voice and choice in coordinated care and services.
  • Maintain documentation required to access and retain OhioRISE benefits.
  • Meet ISBH OhioRISE quality and fidelity standards.
  • Build and maintain knowledge of available community resources.
  • Attend required in-person and virtual training and supervision, including travel.
  • Complete assigned training and maintain required certifications.
  • Follow confidentiality, professional conduct, incident reporting, HIPAA, mandated reporting, and ethical requirements.
  • Use databases and electronic health record systems to document client services.
  • Travel throughout the assigned service area.
  • Perform additional duties as assigned.

Talablar

  • Three years of experience with a high school diploma or equivalent, two years with an associate or bachelor’s degree, or one year with a master’s degree or higher providing services or support to children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare.
  • Knowledge of and experience with High-Fidelity Wraparound preferred; certification is provided upon employment.
  • Two years of experience in coordinated supportive services or care coordination requiring strong efficiency, organization, and attention to detail preferred.
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred.
  • Experience with family and community systems and resources, case management, child and family counseling or therapy, child protection, or child development required.
  • Qualifications and experience working with children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and child-serving systems required.
  • Strong verbal and written communication skills required.
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings required.
  • Ability to protect confidential information required.
  • Ability to follow all safety standards, rules, regulations, and requirements required.
  • Ability to bill Medicaid without restrictions required.
  • Valid driver’s license and ability to operate a motor vehicle required.
  • Appropriate automobile insurance coverage required.
  • Ability to manage deadlines in a fast-paced, high-volume environment required.
  • Ability to work in an internet-based, automated office environment required.
  • Ability to conduct and participate in on-camera video calls and meetings required.
  • Minimum 100 Mbps high-speed internet connection required.
  • Ability to sit for extended periods and work at a computer required.
  • Ability to lift up to 15 pounds occasionally required.
