Bilingual Intake Nurse (RN)

Assess eligibility, safety, and care needs for CareSource members considering managed long-term care. Conduct outreach and coordinate enrollment across the Bronx, Manhattan, and Westchester.

Description

  • Assess prospective members for program eligibility and explain the program to them and their families.
  • Conduct community outreach as needed.
  • Complete initial assessments using the Uniform Assessment System (UAS).
  • Evaluate prospective members for eligibility and safety.
  • Identify care needs and develop initial care plans.
  • Coordinate with clinical teams to support smooth service delivery.
  • Complete enrollment records.
  • Help the community resolve intake questions.
  • Address older members’ developmental, mental, social, and cultural needs in accordance with age-specific requirements.
  • Coordinate with prospective members, families, referral sources, and physicians to support transitions into managed long-term care.
  • Work with Intake/Outreach and clinical teams.
  • Travel regularly to homes, offices, and other public settings.

Requirements

  • Associate degree in Nursing.
  • At least one year of experience in an acute care setting.
  • Current New York State Registered Nurse license.
  • Ability to read, write, and speak English.
  • Knowledge of Medicare and Medicaid regulations.
  • Valid driver’s license, vehicle, and verifiable insurance.
  • Successful completion of a driver’s license record check.
  • Annual influenza vaccination during flu season, October 1 through March 31.
  • Residence within the assigned territory, unless an exception is considered for business needs.
  • Availability to travel more than 50% of the time.
  • Ability to use general office equipment.
  • Bachelor’s degree, home care experience, and managed long-term care intake experience are preferred.
