OhioRISE Care Coordinator – Ohio (Onsite)
Coordinate OhioRISE care teams for youth with complex behavioral health needs. Connect children and families with individualized services across home, school, and community settings.

- Plan and facilitate child and family team meetings.
- Deliver care coordination in families’ homes, communities, or other preferred settings.
- Work with youth, families, natural supports, and providers to identify care needs and available services.
- Support youth and families in exercising voice and choice throughout coordinated care.
- Keep documentation current for accessing and retaining OhioRISE benefits.
- Adhere to ISBH OhioRISE quality and fidelity standards.
- Stay informed about local community resources.
- Participate in required in-person and virtual training, supervision, and travel.
- Complete assigned training and maintain required certifications.
- Follow ISBH requirements for confidentiality, professionalism, and incident reporting.
- Use databases and electronic health record systems to document client information accurately.
- Comply with HIPAA, mandated reporting obligations, and professional ethical standards.
- Travel throughout the assigned service area.
- Complete other assigned responsibilities.
आवश्यकताएँ
- Required experience supporting children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare: three years with a high school diploma or equivalent, two years with an associate or bachelor’s degree, or one year with a master’s degree or higher.
- Hi-Fidelity Wraparound knowledge and experience are preferred; certification is provided upon employment.
- Two years of experience in coordinated supportive services or care coordination is preferred.
- Experience supporting people with autism spectrum disorders and developmental disabilities is preferred.
- Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development is required.
- Required qualifications include experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems.
- Strong verbal and written communication skills are required.
- Must be able to use a personal or company vehicle to deliver services in homes, schools, and community settings.
- Must be able to protect confidential information.
- Must follow applicable safety rules, regulations, and requirements.
- Must be able to bill Medicaid without restrictions.
- A valid driver’s license and ability to operate a motor vehicle are required.
- Appropriate automobile insurance coverage is required.
- Must manage deadlines in a fast-paced, high-volume environment.
- Must be able to work in an internet-based, automated office environment.
- Must be able to participate in on-camera video calls and meetings.
- A minimum 100 Mbps high-speed internet connection is required.
- Must be able to sit for extended periods while working at a computer.
- Must occasionally lift up to 15 pounds.
