Descriere

  • Plan and facilitate child and family team meetings
  • Deliver care coordination in families' homes, communities, or preferred locations
  • Support youth across home, school, and community settings
  • Collaborate with youth, families, natural supports, and service providers to identify care coordination needs and services
  • Ensure youth and families have a meaningful voice and choice in coordinated care and services
  • Maintain documentation needed to access and retain OhioRISE benefits
  • Comply with ISBH OhioRISE quality and fidelity standards
  • Keep current knowledge of available community resources
  • Attend required training and supervision sessions that involve travel
  • Complete assigned training on schedule and maintain required certifications
  • Follow confidentiality, professional conduct, incident reporting, HIPAA, mandated reporting, and ethical standards
  • Use databases and electronic health record systems for client documentation
  • Travel throughout the assigned service area
  • Perform additional duties as assigned

Cerințe

  • Experience serving or supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare: three years with a high school diploma or equivalent, two years with an associate or bachelor's degree, or one year with a master's degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience working with people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family and community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle to provide services in homes, schools, and community settings
  • Ability to protect confidential information
  • Ability to follow safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver's license and ability to operate a motor vehicle
  • Appropriate automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume environment
  • Ability to work in an internet-based, automated office environment
  • Ability to conduct and participate in on-camera video calls and meetings
  • Minimum 100 Mbps high-speed internet connection
  • Capacity for prolonged sitting at a desk and computer work
  • Ability to lift up to 15 pounds
