Bilingual Transitions of Care Nurse Practitioner or Physician Assistant

Provide virtual post-discharge and urgent care for older Devoted Health members as an NP or PA. Coordinate complex chronic care and help reduce hospital readmissions.

Description

  • Conduct comprehensive initial care-transition visits following hospitalization
  • Evaluate medical conditions, medications, functional ability, and psychosocial needs
  • Reconcile medications and optimize therapy in line with clinical guidelines
  • Order prescriptions, laboratory tests, imaging, referrals, home health, and durable medical equipment
  • Educate patients about their conditions and create plans for worsening symptoms
  • Provide acute and virtual urgent care during care transitions
  • Evaluate, diagnose, and treat new or worsening symptoms in medically complex patients
  • Choose appropriate levels of care and escalate cases when necessary
  • Oversee each patient's care during the 30 days after discharge
  • Join interdisciplinary transition-of-care rounds for patients at high risk of readmission
  • Coordinate with case managers, care coordinators, social workers, pharmacists, primary care providers, and specialists
  • Maintain clear communication and ensure patient concerns are escalated appropriately
  • Contribute to clinical education, quality improvement, pilot programs, and EHR improvements
  • Help develop the transitions-of-care program, team culture, and new clinical initiatives

Requirements

  • At least five years of direct patient-care experience, primarily with older adults managing multiple complex chronic conditions across acute and chronic presentations
  • Comfort managing acute episodes of chronic illness and providing ongoing outpatient chronic-disease care
  • Availability to work full time, five eight-hour days per week, Monday through Friday, from 8:30 a.m. to 5:00 p.m.; Central, Mountain, or Pacific time zones preferred
  • Active, good-standing RN and APRN licenses, or an active, good-standing Physician Assistant license
  • Appropriate licensure and good standing in current or future Devoted states
  • Ability and willingness to obtain licenses in required states within 90 days of hire
  • Experience in internal medicine, urgent care, emergency medicine, hospital medicine, post-acute medicine, or geriatrics preferred
  • Telehealth video-visit experience preferred
  • Experience caring for patients after care transitions and recognizing when immediate intervention is needed preferred
  • Experience managing congestive heart failure exacerbations, diabetic emergencies, COPD exacerbations, and hypertensive emergencies preferred
  • Strong interdisciplinary collaboration skills preferred
