OhioRISE Care Coordinator

Coordinate OhioRISE services for youth with complex behavioral health needs. Facilitate family teams and connect children and families with community resources.

Descriere

  • Build and organize family-defined teams for youth receiving OhioRISE services.
  • Develop, lead, and facilitate child and family team meetings.
  • Deliver care coordination in family homes, community settings, or other preferred locations.
  • Collaborate with youth, families, natural supports, and service providers to identify needs and services.
  • Guide teams in defining needs, goals, and action steps.
  • Promote youth and family voice and choice throughout coordinated care and services.
  • Maintain documentation required to access and retain OhioRISE benefits.
  • Comply with ISBH OhioRISE quality and fidelity standards.
  • Maintain current knowledge of available community resources.
  • Attend required in-person and virtual training and supervision sessions, including travel.
  • Complete assigned training and maintain required certifications.
  • Follow confidentiality, professional conduct, incident reporting, HIPAA, and mandated reporting requirements.
  • Use databases and electronic health record systems to document client services.
  • Travel throughout the assigned service area.
  • Complete other assigned duties.

Cerințe

  • Three years of experience supporting children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services with a high school diploma or equivalent; two years with an associate or bachelor's degree; or one year with a master's degree or higher.
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment.
  • Two years of experience in coordinated supportive services or care coordination preferred.
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred.
  • Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development required.
  • Experience working with children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems required.
  • Strong verbal and written communication skills.
  • Ability to use a personal or company vehicle to deliver services in homes, schools, and community settings.
  • Ability to protect confidential information.
  • Ability to follow safety rules, regulations, and requirements.
  • Ability to bill Medicaid without restrictions.
  • Valid driver's license and ability to operate a motor vehicle.
  • Appropriate automobile insurance coverage.
  • Ability to manage deadlines in a fast-paced, high-volume environment.
  • Ability to work in an internet-based, automated office environment.
  • Ability to conduct and participate in on-camera video calls and meetings.
  • Minimum high-speed internet connection of 100 Mbps.
  • Ability to sit for extended periods and work at a computer.
  • Ability to lift up to 15 pounds and comply with HIPAA, mandated reporting, and professional ethical requirements.
