Field Registered Nurse Case Manager - Houston Hybrid

CVS Health is seeking a Registered Nurse Case Manager to coordinate assessments, individualized care plans, referrals, and hospital-based services. The role supports members through onsite collaboration with providers and multidisciplinary teams at Houston Methodist Hospital.

Apraksts

  • Act as a liaison among members, families, employers, providers, insurers, and healthcare professionals
  • Coordinate case management for members with catastrophic or chronic conditions across the care continuum
  • Arrange consultant referrals, home care, community resources, and alternative care settings
  • Engage with members by phone and in person while working onsite at Houston Methodist Hospital
  • Meet with members during inpatient hospital stays
  • Evaluate medical, functional, and vocational needs
  • Create and carry out individualized care plans
  • Communicate with providers, attorneys, employers, and insurance carriers
  • Maintain timely, accurate records of case management activities
  • Partner with internal multidisciplinary teams
  • Conduct outreach to treating physicians and specialists
  • Deliver member education and preventive guidance
  • Follow applicable laws, regulations, and payer requirements
  • Track member progress and revise care plans as needed
  • Develop proactive approaches for complex needs and short- and long-term wellness outcomes

Prasības

  • Hold an active, unrestricted Registered Nurse license in Texas
  • Bring at least three years of clinical experience, preferably with Medicare and commercial populations
  • Work independently in a hospital-based setting
  • Demonstrate strong analytical, problem-solving, organizational, and communication abilities
  • Use Microsoft Word, Excel, Outlook, PowerPoint, and proprietary clinical systems effectively
  • Navigate multiple systems and perform keyboard-based computer tasks efficiently
  • Have reliable transportation
  • Meet onsite hospital work requirements at Houston Methodist Hospital for up to 50–75% of the role
  • Associate degree in Nursing with equivalent clinical experience is required
  • Have at least three years of experience in care management, discharge planning, or home health coordination
  • Hold a national certification such as CRC, CDMS, CRRN, COHN, or CCM
  • Comfortably configure and use multiple monitors while working across applications
  • Communicate through email, calendar invitations, Teams messaging, and virtual meetings
  • Have experience with Microsoft Office 365 or comparable Google Workspace tools
  • Access secure systems such as VPN and electronic health record portals
  • Manage strong passwords and identify suspicious emails or links
  • Resolve common technical issues independently
  • Be willing to develop new skills as workplace technology and processes evolve
