OhioRISE Care Coordinator (Onsite, Ohio)

Coordinate OhioRISE services and collaborative care teams for youth with complex behavioral health needs. Support families by connecting them with community resources and coordinating individualized care across homes, schools, and community settings.

Description

  • Lead and facilitate child and family team meetings from planning through follow-up
  • Deliver care coordination in families’ homes, communities, or other preferred settings
  • Work with youth, families, natural supports, and service providers to identify care needs and available services
  • Ensure youth and families can actively express their preferences and choices in coordinated care
  • Maintain the records needed to obtain and continue OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity standards
  • Stay informed about community resources
  • Attend required in-person and virtual training and supervision sessions, including travel
  • Complete assigned training and keep required certifications current
  • Follow ISBH policies for confidentiality, professional conduct, and incident reporting
  • Use databases and electronic health record systems to document client information accurately
  • Comply with HIPAA, mandated-reporting obligations, and professional ethics
  • Travel throughout the assigned service area
  • Carry out other duties as assigned

Requirements

  • Three years of experience supporting children and families involved with behavioral health, child welfare, developmental disabilities, juvenile justice, or related public-sector human services or behavioral healthcare with a high school diploma or equivalent; two years with an associate or bachelor’s degree; or one year with a master’s degree or higher
  • Hi-Fidelity Wraparound knowledge and experience preferred; certification is provided upon employment
  • Two years of experience in coordinated supportive services or care coordination preferred
  • Experience supporting people with autism spectrum disorders and developmental disabilities preferred
  • Experience with family and community systems, community resources, case management, child and family counseling or therapy, child protection, or child development
  • Qualifications and experience supporting children and families affected by serious emotional disturbance, trauma, co-occurring behavioral health disorders, and involvement with child-serving systems
  • Strong verbal and written communication skills
  • Ability to use a personal or company vehicle to deliver services in homes, schools, and community settings
  • Ability to protect confidential information
  • Ability to follow safety rules, regulations, and requirements
  • Ability to bill Medicaid without restrictions
  • Valid driver’s license and ability to operate a motor vehicle
  • Appropriate automobile insurance coverage
  • Ability to manage deadlines in a fast-paced, high-volume environment
  • Ability to work in an internet-based, automated office environment
  • Ability to conduct and participate in on-camera video calls and meetings
  • Minimum 100 Mbps high-speed internet connection
  • Ability to sit for extended periods while working at a desk and computer, and lift up to 15 pounds when needed.
